If you do not understand the requirements of the Third-party Evidence Report or you are unable to complete it in your workplace, please contact your Trainer/Assessor.
Instructions: Please familiarise yourself with the criteria within the document before forwarding to your workplace leader. Your workplace leader must complete this form over the next few months while observing you in the workplace . Once your workplace leader has completed the form and returned to you by the due date, upload the tool for review by your Trainer/Assessor.
It is your responsibility to ensure your workplace leader completes and returns the form to you by the due date specified within the document.
